Healthcare is the largest agency demand we track and the vertical where the GEO shelf is least like the rest: when we asked ChatGPT and Perplexity on 5 September which agencies do AI search optimisation for healthcare companies and clinics, the engines named twenty-two agencies between them, most of them long-standing medical-marketing shops that have added an AI line, and cited First Page Sage’s healthcare ranking and a Singapore AI-Overview specialist in the same breath. So we scored the ones the engines named on the criteria below.
how this list was built
The candidates are the agencies that ChatGPT and Perplexity actually named on 5 September 2026 when asked for GEO or AEO agencies for healthcare, plus the healthcare-marketing incumbents on the ranking the engines cited most. That produced sixteen names; ten made the cut because we could read a public page describing their healthcare AI-search work. Not scored: one large healthcare agency whose site blocked our reader on every page, one AI-Overview specialist with no healthcare evidence on its own site, a health PR network whose AI offer we could not find on its pages, and one domain that redirects to a video channel.
The criteria and weights are the six from the main list: published evidence 30%, engine coverage 20%, disclosed cadence 15%, vertical depth 15%, pricing transparency 10%, own organic footprint 10%. Each is scored 0, 1 or 2 from public evidence only. Footprint thresholds: under 500 monthly organic visits scores 0, 500 to 10,000 scores 1, over 10,000 scores 2. Where we come last: footprint, at zero. Our organic footprint in the US is zero: the domain is new. On vertical depth we score 0 — we have no completed healthcare audit, and seven agencies on this list are healthcare-only with named clients. Re-weight vertical depth to 30% and footprint to 20% and Intrepy, Healthcare-only shops and First Page Sage lead. We are first only because we publish the method and the numbers, and a clinic that wants a hundred medical logos should read the next entries first.
How the measurements were taken. Organic figures are a DataForSEO bulk traffic estimate for the US, synonym expansion off, pulled on 5 September 2026. Shelf observations are a live prompt run on the same day on ChatGPT (gpt-5.5, web search on) and Perplexity (sonar-pro), logged verbatim. Everything else is read off the agencies’ public pages on that date. No agency was contacted, nobody paid to be here, and there are no affiliate links on this page.

what the list actually shows
Three patterns repeat, and they matter more than the order.
Healthcare GEO is being sold by medical-marketing agencies. Seven of ten agencies on this list are healthcare-only and have been for years; their AI-search offer is a new service line beside paid media and web design. Their client evidence is real and their disclosure of engines and cadence is, on the pages we read, mostly absent. A clinic should ask the AI question specifically: which engines, how often, and what the last dated series looked like.
The engines verify against registers, so the agencies that mention them lead. In our live patient test the engines cited the National Joint Registry, CQC data and peer-reviewed evaluations before any clinic page. Only two agencies on this list describe credential and trust-signal work in those terms; the rest describe content and rankings.
Footprint still does not predict the shelf. Stethon has around forty organic visits a month and names five engines; Healthcare Success has five thousand and names none. Both were on the shelf. The page that answers the question, not the size of the site, decided who was there.
how to use this if you are hiring
Ask any agency on this list, including us, for three things: the prompt panel they would measure you on, the engines they run it against, and the last delta they reported to a client in your vertical. An agency that sells AI visibility and cannot show a dated series has not measured it. how do I choose a GEO agency for a clinic or health brand?
Ask what the engines currently tell a patient in your specialty and city, and which sources they checked to decide. In our test on 5 September the answer was registries, peer-reviewed evaluations, Reddit and review platforms before any clinic site. Then ask which engines the agency measures, how often, for a dated series from a clinic in your specialty, and how clinician review and compliance work before a page goes live. An agency that talks about rankings alone is selling medical SEO with a new name.

Healthcare-only depth is a real advantage on procedure content and compliance; it is not, in our measurements, an advantage on the entity and credential layers, where the mechanics are the same in law, finance and healthcare.

Not blindly, and that includes ours. The engines fill their shelves from self-published rankings; First Page Sage’s healthcare list was the first source Perplexity cited. What separates a usable list is stated criteria, visible scores and a named weakness. Ours has two on this page — footprint and healthcare depth, both zero — and the table lets you re-order it around either.
